Your practice can join Premier Dentists and get listed by simply filling out the form below. Upon completion of the form click the submit button and you will then be presented with an agreement form already filled out with your practice details.

All you then need to do is print out the agreement, sign it, scan it and then email it back to us. It really is that simple.

If we already have 5 practices listed for your county we will then place you on the waiting list and as soon as a position becomes available we will let you know before activating your account.
